Patients are fully responsible for charges incurred at our office, and payment is due at the time of treatment. We accept Visa, MasterCard, checks, and cash.

As a courtesy, our staff will file insurance claims for patients and wait up to ninety days for payment from an insurance company. After ninety days, if insurance does not pay, responsibility falls to the patient. We are participating providers with Delta Dental and AnthemInsurance.

Our time is very valuable, as is yours. Please call the office twenty-four hours in advance to reschedule an appointment. A $75.00 fee will be charged to your account for missed appointments or reschedules with less than twenty-four hours’ notice.
